Student Enrollment Instructions

We are excited that you have taken the first step in becoming a Blackhawk, and look forward to you and your student joining our school community.  

All of our school campuses have an active presence on Social Media, and we encourage each member of your family to follow the district and your students school Facebook page, as well as look to our district web page for all of the resources and information you will need while you have a student in our district. 

Choose whether you would prefer to complete the application by hand or by using a computer.

Option A – Using a Computer/Device:

  1. Download the New Student Enrollment Application here.

  2. Fill out the PDF application and save it on your computer.

Note: In order to fill out the document on your computer, you will need to to download PDF and open it directly with Adobe Acrobat. Opening the file in your browser will not let you edit it.

 

  1. Upload the application and other required documentation here

Option B – By Hand:

  1. Download the New Student Enrollment Application here and print it yourself, or stop by Student Services office located at 781 West Pickens (on the East side of Pea Ridge Junior High School) to pickup an already printed copy.

  2. Fill out the forms by hand.

  3. Upload the application and other required documentation here or, if you are choosing to return your documents in person, we still ask that you complete the enrollment quick start online, and then skip the upload portion.  You can bring them in to Student Services office located at 781 West Pickens (on the East side of Pea Ridge Junior High School).

Please make sure that you have all of your documents, saved and ready for upload prior to starting the enrollment quick start online.  If you are choosing to return your documents in person, we still ask that you complete the enrollment quick start online, and then skip the upload portion. 

If at any time, you are needing help with the Enrollment Process, please reach out to our Student Services Office by phone at 479-451-0098, or stop by our offices which are located at 781 West Pickens (on the East side of Pea Ridge Junior High School).
